Community Health Worker Job Description
Non-Exempt Full-time (35 hours/week)
Job Overview: The Community Health Worker is responsible for assisting clients in navigating the health care system, educating about appointments and follow-up needs, troubleshooting barriers to care, and coordinating assistance from and linking clients to community support services as applicable. This position works closely with medical providers, primary care teams, and social service agencies to provide short-term care coordination. Represents the target population and promotes individual and community advocacy. Works in community-based settings and conducts visits in clients’ homes and neighborhoods. This position is based primarily in the field with at least one day per week spent in the office.
Major Job Responsibilities:
● Develops individual client care coordination service plans and documents all activity.
● Encourages and supports clients and their families, in culturally appropriate ways, to make concrete steps toward promoting their health and managing any chronic illnesses.
● Calls patients to inform/remind them of appointments and transportation plans.
● Monitors the delivery and outcome of services provided to clients by medical facilities, social services agencies, and community resources.
● Identifies gaps in health services and barriers to care and develops program plans to meet these needs.
● In collaboration with the media team, creates health education and awareness social media content that is culturally relevant and responsive to community needs.
● Keeps abreast of changes to health insurance and the health care system in order to educate patients and community members and links to application assistance.
● Helps clients gain confidence in navigating the health care system.
● Completes documentation required by appropriate funding sources and participates in program evaluation.
● Participates in staff and community meetings, including local and statewide Community Health Worker meetings.
● Organizes monthly health education workshops and other events, such as health fairs, in collaboration with Community Ambassadors and Navigators and ensures that pre-/post-evaluations are conducted for all workshops and events.
Job Requirements: High School degree or GED required. Valid driver’s license and ability to drive to clients homes. Cultural competence in working with various East African ethnic groups recommended. Strong oral and written communication skills necessary. Ability to compile data related to grant outcomes, and complete paperwork thoroughly, accurately and timely. Familiarity with computers, internet and e-mail required. Fluent in English and Swahili and/or Kinyarwanda.
Salary: $25/hr 35 hrs/week
